Secondary Headaches: These are symptoms or side effects of an entirely separate underlying condition or physical injury. A cervicogenic headache is classified as a secondary headache because it is caused by a neck-related structural, muscular, or neurological trigger. 

Cervical Spine Structure  

The upper neck nerves (C1, C2, and C3) connect to the brain stem. When these nerves are disturbed, the brain reads it as head or face pain instead of neck pain.

What Causes Cervical Headaches?

  • Arthritis (Cervical Osteoarthritis): Degeneration of neck joints or discs can cause inflammation and pressure on nerves that radiate to the head.
  • Nerve compression: Pinched nerves in the upper cervical spine can cause pain radiating from the neck into the scalp or around the eyes.

How Is a Cervicogenic Headache Diagnosed? 

A cervicogenic headache (CGH) is diagnosed through a combination of a detailed clinical interview, a hands-on musculoskeletal evaluation, and targeted diagnostic testing. Because its symptoms overlap heavily with other headache types, doctors focus on pinpointing the underlying neck dysfunction to make an accurate diagnosis.  

A clinical diagnosis is typically achieved using four distinct components:  

1. Medical History  

Your healthcare provider will conduct a thorough interview to understand your symptoms, headache patterns, and potential triggers. Key indicators they look for include: 

  • Unilateral (one-sided) head pain that does not switch sides. 
  • Accompanying shoulder, arm, or non-radicular neck pain.  

2. Physical Examination 

A doctor or physical therapist performs a hands-on physical and neurological assessment. This includes:  

  • Palpation: Pressing on specific areas of the upper cervical spine, the base of the skull, and shoulder muscles to check for deep tenderness or active trigger points.  
  • Replication: Determining if applying pressure to the neck or applying certain sustained postures can recreate your typical headache pain.  
  • Neurological Screening: Checking your reflexes, strength, and sensation to rule out nerve compression in the arms or face.  

3. Range of Motion (ROM) Testing

  • Imaging: X-rays, MRIs, or CT scans may be used to rule out more serious underlying structural issues, such as spinal tumors, disc herniations, or vertebral artery dissections.  
  • Diagnostic Injections (The Gold Standard): If a physical exam leaves the diagnosis in question, a specialist may perform a fluoroscopy-guided anesthetic nerve block (e.g., of the greater occipital nerve or cervical facet joints). Complete relief of both the neck pain and the headache following the injection serves as a definitive confirmation that the headache originates from the neck.
